I will collect personal details as well as information in relation to your health from the initial form that you fill in and supply to me. I will store this information electronically on a private laptop with password protection. Consultation notes are stored on computer software package on a secure sever. Any information on paper that I hold on you eg. appointment notes will be filed in a locked case and stored in my clinic room, which is locked when I am not there.
I will use this information in order to contact you about class times or new services that might be of interest to you. Having this information is also important to ensure your safety and suitability of certain exercises for you.
I will keep this information on file for as long as you use my services and for 7 years after, which is in line with EU regulations for handling medical information. For children data will be held for 7 years after their 18th birthday.
I will not share your personal information with any third party without your prior consent. The only time that I think it might be necesssary to pass on this information would be if you required referral to another health or fitness professional. You will be involved in the decision making process as to whether this referral is necessary.
By taking part in our group classes you are giving pre-consent to have occassional photos or videos taken which may be used online or in flyers for marketing purposes. I will make you aware at the time of use prior to publication and you will have the option to withdraw your consent. You also have the choice to accept or reject online ‘tags’ of yourself by changing your own social media settings.

I aim to action your request within 14 days. If you want changes made to your information please fill up another Health History form or you should contact me.
I will notify the Data Protection Commissioner of a breach of GDPR within 72 hours of becoming aware of it, unless it is unlikely to result in a risk to your rights and freedoms. I will also notify you where a breach is likely to result in a high risk to your rights and freedoms without undue delay.
